ICF/IID SERVICES
Persons Served
The ICF/IID program serves adults from 18 to 70 years of age with dual DSM-5 diagnoses of Intellectual Disability and Mental Illness.
Referral and Admission Packets
NOVA’s ICF/IID program accepts Referrals through the Consumer Affairs Coordinator from Parents / Guardians, Physicians, Behavioral Health Professionals, Other Providers and MCO/LME staff. Once the Referral is made and the completed Application Packet is received NOVA staff schedule a visit and assessment at the Applicant's current residence or at one of our sites. NOVA’s Team makes an Admission decision and immediately informs all Stakeholders. If the Admission decision is favorable NOVA’s Consumer Affairs Coordinator expedites all admissions logistics.
